Sacopee Valley Health Center Joins Community Care Partnership of Maine ACO

Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of 12 federally qualified health centers and three community hospitals throughout Maine, is pleased to announce that Sacopee Valley Health Center in Porter has joined their membership. CCPM member organizations are committed to collaboration, resource sharing, and the implementation of best practices for improving the patient and care team experience, achieving high-quality clinical outcomes, and managing costs.

“Becoming part of CCPM allows Sacopee Valley Health Center to further improve the health of the communities we serve in southern Maine in the most cost-effective and transformative manner,” said Lynn Kearney, Executive Director.

Sacopee Valley Health Center is located in Porter, Maine and serving Carroll County in New Hampshire and three counties and ten rural communities in Maine; providing approximately 24,000 patient visits each year.
Founded in 1976, the health center is a private, federally qualified health center specializing in responsive trauma-informed, integrative primary care; a team of primary care and behavioral health clinicians working with patients and families to provide compassionate patient-centered care. The health center provides care on a sliding fee scale, regardless of ability to pay.

“Community Health Centers are woven into the fabric of the communities they serve – they truly are the neighborhood doctor’s office for so many Maine people,” CCPM Chief Executive Officer Mary Prybylo, RN, MSN, FACHE, said. “Health centers historically have focused on the social determinants of health and improving population health, so it is a natural fit for Sacopee Valley Health Center to join our efforts to transform the delivery of healthcare through meaningful sharing and accountability for the health of our patients.”

CCPM’s member organizations collectively serve over 150,000 patients. CCPM is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health, and treatment of at-risk populations with chronic disease.
