Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of 11 federally qualified health centers and three community hospitals throughout Maine, is pleased to announce that Eastport Health Care, Inc. has joined their membership. CCPM member
organizations are committed to collaboration, resource sharing, and the implementation of best practices for improving the patient and care team experience, achieving high-quality clinical
outcomes, and managing costs.

“EHC is honored to join the ranks of CCPM member an organization fully aligned with our commitment and orientation to providing patient centered high quality compassionate cost effective care, in an environment that supports responsive listening and collaborative services,” said Holly
Gartmayer-DeYoung, Chief Executive Officer.

Eastport Health Care, Inc. is located in Eastport, Maine and serves the rural communities of Washington County, including Eastport, Machias, and Calais; providing care to approximately 5,000
patients each year.

Founded in 1978, the federally qualified health center specializes in comprehensive primary family healthcare, routine and preventative dental care, general and surgical podiatry and behavioral healthcare services; a team of primary care and behavioral health clinicians work with patients and families to provide compassionate patient-centered care. The health center provides exceptional care to all residents, regardless of ability to pay.

“Community Health Centers are woven into the fabric of the communities they serve – they truly are the neighborhood doctor’s office for so many Maine people,” CCPM Chief Executive Officer Mary Prybylo, RN, MSN, FACHE, said. “Health centers historically have focused on the social
determinants of health and increasing health care access in our communities, so it is a natural fit for Eastport Health Care, Inc. to join our efforts to transform the delivery of healthcare and improve population health through meaningful shared accountability.”

CCPM’s member organizations collectively serve over 180,000 patients. CCPM is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health, and
treatment of at-risk populations with chronic disease.
