Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of 11 federally qualified health centers and three community hospitals throughout Maine, is pleased to announce that Health Access Network in Lincoln has joined their membership. CCPM member organizations are committed to collaboration, resource sharing, and the implementation of best practices for improving the patient and care team experience, achieving high-quality clinical
outcomes, and managing costs.

By joining this effort, Health Access Network will be able to grow not only within the community we serve, but also within the health care environment,” said Nicole Morrison, RN, Chief Executive Officer.

Health Access Network is located in Lincoln, Maine and serves the rural communities of Lincoln, Medway, Millinocket, West Enfield and Lee; providing care to approximately 12,000 patients visits each year.

Founded in 2003, the federally qualified health center specializes in comprehensive primary healthcare, dental care, podiatry and behavioral healthcare services; a team of primary care and behavioral health clinicians work with patients and families to provide compassionate patient-centered care. The health center provides exceptional care to all residents, regardless of ability to pay.

“Community Health Centers are critical deliverers of health care in our communities– they truly are the neighborhood doctor’s office for so many Maine people,” CCPM Chief Executive Officer Mary Prybylo, RN, MSN, FACHE, said. “Health centers historically have focused on the social determinants of health and improving population health, so it is a natural fit for Health Access Network to join our efforts improve health outcomes for the communities we serve through meaningful collaboration and shared accountability for the health of our patients.”

CCPM’s member organizations collectively serve over 180,000 patients. CCPM is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health, and treatment of at-risk populations with chronic disease.
