Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of 12 federally qualified health centers and three community hospitals throughout Maine, is pleased to announce that Harrington Family Health Center has joined their membership.  CCPM member organizations are committed to collaboration, resource sharing, and the implementation of best practices for improving the patient and care team experience, achieving high-quality clinical outcomes, and managing costs.

Harrington Family Health Center is located in Harrington, Maine and serves the surrounding rural communities, providing care to more than 3,000 patients each year.

Founded in 1984, the federally qualified health center specializes in providing comprehensive medical, dental, podiatry and behavioral healthcare services. Harrington’s team of primary care and behavioral health clinicians work with patients and their families to provide compassionate, patient-centered care to residents across Washington County, regardless of ability to pay.

“Health centers like Harrington are woven into the fabric of the communities they serve – they truly are the neighborhood doctor’s office for so many Maine people,” CCPM Chief Executive Officer Mary Prybylo, RN, MSN, FACHE, said. “Health centers historically have focused on the social determinants of health and improving population health, so it is a natural fit for Harrington Family Health Center to join our efforts to transform the delivery of healthcare through meaningful sharing and accountability for the health of our patients.”

CCPM’s member organizations collectively serve over 185,000 patients. CCPM is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health, and treatment of at-risk populations with chronic disease.
