Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of fourteen federally qualified health centers and three community hospitals throughout Maine, is pleased to announce that Islands Community Medical Services, Inc. has joined their membership. CCPM member organizations are committed to collaboration, resource sharing, and the implementation of best practices for improving the patient and care team experience, achieving high-quality clinical outcomes, and managing costs.

“This unique opportunity to collaborate with other FQHCs in the state and the other members of CCPM will improve the health of our patients through increased care coordination and by sharing best practices,” said Dinah Moyer, Executive Director of Islands Community Medical Services, Inc.

Islands Community Medical Services, Inc. is located on Vinalhaven, Maine and serves its year-round population and summer visitors, providing care to about 1,200 patients each year.

The federally qualified health center specializes in providing comprehensive medical, dental, and behavioral healthcare services as well as physical therapy. Island’s team of primary care and behavioral health clinicians work with patients and their families to provide compassionate, patient-centered care to residents of Vinalhaven, regardless of ability to pay.

CCPM’s member organizations collectively oversee nearly 80,000 lives. CCPM is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health, and treatment of at-risk populations with chronic disease.
