Community Care Partnership of Maine (CCPM), an Accountable Care Organization comprised of fifteen federally qualified health centers and three community hospitals throughout Maine, is pleased to announce that St Croix Regional Family Health Center has joined their membership. CCPM member organizations are committed to collaboration, resource sharing and the implementation of best practices for improving the patient and care team experience, achieving high-quality clinical outcomes and managing costs.

“Community Health Centers provide economic engines that drive innovation and quality in today’s health care climate,”, said Corinne LaPlant, Executive Director of St Croix Regional Family Health Center (SCRFHC) in Princeton, Maine. “Together, with the support of our Board of Directors, we have successfully partnered with other health centers across Maine through a collaboration with Community Care Partnership of Maine. These partnerships enhance care outcomes, improve costs and expand access to health care for rural communities.”

draft7SCRFHC, which was founded in 1991 by a group of concerned community members with the vision and passion to bring care closer to home, continues to serve the rural communities from north of Calais to southern Aroostook county, providing care to over 2800 community members each year. The health center provides compassionate, patient-centered direct care services including primary care, behavioral health, podiatry, dental and substance use disorder services. In addition, supportive services such as prescription and health coverage assistance are offered to help patients and their families when barriers to care are identified, regardless of ability to pay.

“Health Centers like St Croix are woven into the fabric of the communities that they serve – they truly are the neighborhood provider’s office for so many Maine people.” CCPM Chief Executive Officer Mary Prybylo, RN, MSN, FACHE said. “Health centers historically have focused on the social determinants of health and improving population health, so it is a natural fit for SCRFHC to join our efforts to transform the delivery of healthcare through meaningful sharing and accountability for the health of our patients.”

CCPM’s member organizations collectively oversee care for nearly 80,000 lives. CCPM is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health and treatment of at-risk populations with chronic disease.
