  • Sandy Nesin
    Esq.
    Chief Executive Officer
    Sandy Nesin, Esq. serves as Chief Executive Officer for CCPM and is the Vice President of Operations at St. Joseph Healthcare. Sandy joined St. Joseph Healthcare in May 2016. A graduate of the University of Maine (B.A), and University of Maine School of Law (J.D.), Sandy has experience working in both health care and health insurance operations. While attending Law School, Sandy was awarded an Equal Justice Works Student Fellowship for her work with the Maine Medical Association in providing education and support to primary care practices and providers regarding passage of the Affordable Care Act. Sandy currently serves as a Board Member of United Way of Eastern Maine and has held several past board positions for not-for-profit organizations across the state.
  • Mary Prybylo
    RN, PhD, MSN, FACHE
    Senior Research Advisor
    Mary Prybylo, RN, MSN, FACHE serves as CCPM’s Senior Research Advisor. Mary recently retired from her position as the President of St. Joseph Healthcare in Bangor. Mary brings to her role a passion for innovation and patient-centered care, along with extensive experience in both healthcare leadership and administration. Prior to joining St. Joseph Healthcare, Mary served as the Chief Operating Officer at Waterbury Hospital Health Center, where she was responsible for operations, physician partnerships and strategic initiatives that supported the delivery of patient care in both inpatient and outpatient settings. Mary held several positions including the Director of Community Care Management and Vice President of Managed Care and Operations within the Anthem Blue Cross and Blue Shield of Connecticut system, as well as Vice President for Patient Care Services at St. Mary’s Hospital in Waterbury, CT.
  • Timothy Clifford

    Chief Medical Officer
    Dr. Clifford serves as CCPM’s Chief Medical Officer and also works as a family physician at Bucksport Regional Health Center. Tim is an avid learner with an insatiable thirst for knowledge. The medical profession fits that trait perfectly. He has been a primary care provider for decades, having spent most of his career at BRHC. He previously served as the Medical Director for MaineCare and Goold Health Systems. Dr. Clifford graduated from Boston College with a degree in psychology major, and went on to earn his medical degree from Tufts University School of Medicine. He is married to wife, Nancy, a dentist, and has two grown children. In his spare time, he enjoys running, reading mysteries, and gardening.
  • Mary Butler-Fleming
    Chief Operating Officer
    Mary Butler-Fleming serves as the Chief Operating Officer of Community Care Partnership of Maine (CCPM). Since joining the organization in 2018, she has held multiple roles, including serving as the organization’s Compliance Official. Mary earned dual degrees in Public Health and Business, Entrepreneurship, and Organizations from Brown University. A Bangor native and lifelong Mainer, she is deeply committed to advancing public health and community well-being, serving on the Boards of the Maine Public Health Association and the Hilltop School in Bangor. In 2025, she was recognized by MaineBiz’s 40Under40. Mary has contributed to national and regional discussions on value-based care policy and has presented at forums such as the National Association of ACOs (NAACOS), the New England Rural Health Association, and the Maine Primary Care Association. She resides in the Bangor area with her husband and their two young sons. In her free time, she enjoys thrifting, cooking, and spending time with her family.
  • Kim Alexander

    Chief Clinical Operations Officer
    Kim Alexander, RN, serves as CCPM’s Chief Clinical Operations Officer. She joined CCPM in 2019 as the Director of Quality for CCPM, and stepped into the Chief Clinical Operations Officer role in 2024. Kim is a dedicated nurse leader committed to ensuring Mainers have access to high quality, patient-centered care. Kim has over 20 years’ experience as a Registered Nurse, and previously served as the Vice President of Physician Practices at Mayo Regional Hospital in Dover-Foxcroft as as a nurse leader at Acadia Hospital in Bangor. Kim’s effectiveness as a nurse leader is driven by her passion for innovation, integrity, authenticity, and sense of humor. Kim is a proud alumnus of the University of Maine, where her attained her BSN. Kim lives in Orrington with her two sons, Nate and Noah. When she is not working, Kim enjoys spending time at her family camp in Island Falls, traveling and gardening.
  • Adam Richards
    Director of Value-Based Contract Operations
    Adam comes to us from Anthem Blue Cross Blue Shield, where he spent eighteen years in various roles. Most recently, he served as a Provider Collaboration Director, leading Maine’s value-based care programs. His deep experience in this space will be a tremendous asset to our team. Adam lives in Windham with his wife and two children. As a family they enjoy spending time by the water during the summer. In his free time, Adam also enjoys skiing and mountain biking.
  • Alycia Botting
    Pre-Visit Planning Medical Assistant
    Alycia Botting joined CCPM as a Pre-Visit Planning Medical Assistant in July 2024. She is a Certified Medical Assistant. Prior to her work at CCPM, Alycia worked as an MA at Penobscot Valley Hospital. She rotated between the specialty, surgical, behavioral health and family medicine clinics. In her free time, Alycia enjoys spending time with family, traveling and going to the beach.

    Pre-Visit Planning Medical Assistant
    Angel Ireland joined CCPM as a Pre-Visit Planning Medical Assistant in the fall of 2021. She is a Certified Clinical Medical Assistant bringing several years of work experience in the healthcare field. Prior to her time with CCPM, she worked as a medical assistant for primary care, walk in care and occupational medicine. Angel is proficient in HIN Analytics and IMMPACT and has signficant knowledge of various EMR systems. When she is not working, Angel enjoys camping, traveling, reading and spending time with her family.

    Pre-Visit Planning Medical Assistnt
    Cassandra Smith joined CCPM as a Pre-Visit Planning Medical Assistant in July 2024. She earned her Clinical Medical Assistant Certificate through Northeast Technical Institute. Prior to joining CCPM, she worked as a Medical Assistant at PCHC and Northern Light for over seven years. In her free time, Cassandra enjoys long distance running, spending time with her family and reading.

    Executive Administrator
    Danielle Brown serves as CCPM’s Executive Administrator. She graduated with her BS in Health Care Studies and went on to obtain her MBA at Husson University. In 2023, Danielle earned her Professional Administrative Certification of Excellence (PACE) Certification. Prior to joining the CCPM team, Danielle worked at various hospitals and healthcare facilities in the Bangor area providing administrative support to staff and patients. She demonstrates a high level of expertise and knowledge in the administrative field. Danielle is a member of the American Society of Administrative Professionals. She lives in the greater Bangor area with her husband. In her free time, Danielle enjoys reading, cuddling with her cats and spending time with her family.

    Clinical Quality Manager
    Dusty Hillman serves as CCPM’s Clinical Quality Manager. Dusty holds a Bachelor Degree in Interdisciplinary Studies with a focus on Business Administration. A veteran Medical Assistant, Dusty became a Registered Medical Assistant after attending Anne Arundel Community College in 2000. Prior to joining the CCPM team, Dusty worked at Northern Light Husson Family Practice for seven years as a medical assistant and phlebotomist. In 2007, she began her work at Penobscot Community Health Care working in the school based health center as a coordinator of services. In this role, Dusty oversaw data collection and reporting and served as a facilitator for the Student Health Advisory Board. In 2016, Dusty was recognized as the non-clinical employee of the year in her department at PCHC. Dusty resides just outside the greater Bangor area with her husband, Andy. In her spare time, Dusty enjoys reading and playing golf.
  • Erin Leighton

    Data Analytics & Information Systems Manager
    Erin Leighton serves as CCPM’s Data Analytics & Information Systems Manager. Erin received a BA in English in 2005 from the University of Maine and an Associate of Science in Health and Information Management in 2013 from Beal University. In 2015, she launched her healthcare career as a Data Analyst at Northern Light Health in the Credentialing and Provider Data Management Department, eventually serving as the department’s Database Administrator. In 2018, she was hired as a Quality Improvement Specialist at CCPM. Prior to her current role, Erin held various positions at CCPM as its Quality Data Analyst and Information Systems & Database Administrator. Erin earned her first credential from the American Health Information Management Association (AHIMA) in 2014 when she passed the Registered Health Information Technician (RHIT) exam. Later in 2021, Erin earned the advanced version of the credential when she passed the Registered Health Information Administrator (RHIA) exam. More recently, Erin passed the Certified in Healthcare Privacy and Security (CHPS) certification from AHIMA in May 2023. Erin enjoys gardening and reading in her spare time. She lives in Brewer with her husband, Corey, and her son, Alex, along with their two cats and two dogs.

    Director of Development & Grants
    Jana Richards joined the CCPM team as the Development & Grants Manager in 2022 and was promoted to CCPM’s Director of Development & Grants in 2025. Jana graduated from the University of New England with a Bachelor of Science in Medical Biology. Prior to joining the CCPM team, Jana worked as a public health professional with over 10 years of experience working in Maine non-profits implementing evidence-based practices in public health, with a focus on behavioral health and health equity. Jana brings her expertise in project management, grants writing and management to the team. Jana is also a leadership team member of SoPo Unite, a Drug-Free Community Coalition in South Portland. She, her husband and young son live in Lincoln County Maine. She enjoys spending time in nature with her family, hiking, gardening, swiming and kayaking. One of her favorite pastimes is to drink a hot cup of coffee with a good book in front of a fireplace.

    Senior Medical Advisor, Workforce Development & Rural Health Innovation
    Jonnathan Busko, MD joined CCPM as an Innovation Advisor in the fall of 2023. He has been an ED Physician since 2005. In addition to his role at CCPM, he is the Medical Director for Care Delivery Transformation and Associate Medical Director of the ED at St. Joseph Healthcare. He is passionate about innovation in healthcare, often advocating for new and innovative ways to improve access to care in the most rural regions of our state. Dr. Busko resides in Bangor. In his spare time, he enjoys skiing, hiking, sailing and kayaking.
  • Katie Butler
    Director of Research and Innovation
    Katie Butler joined CCPM as the Director of Research & Innovation in 2023. She oversees the operational aspects of the Controlled Substance Stewardship Program and other innovative or pilot projects. Before CCPM, Katie spent two and a half years at St. Joseph Healthcare as a Project Manager and then as Director of Operations for the Schmidt Institute. She graduated from Tufts University in 2021 with a degree in Community Health. Katie resides in Portland with her family and dog, a wirehaired pointing griffon named Bruce. In her free time, she enjoys cooking, thrifting, decorating her home, and Pilates.

    Kayla Dube joined CCPM in 2022 as a Pre-Visit Planning Medical Assistant. She is a certified Registered Medical Assistant. Prior to her work at CCPM, Kayla worked for many years as a medical assistant at Health Access Network. She has significant knowledge working in various EMR systems and an exceptional understanding of quality measures. In her free time, Kayla enjoys camping, fishing, and spending time at the lake with family & friends.

    Kristen Worster joined the CCPM team as a Pre-Visit Planning Medical Assistant in 2022, and was promoted to Quality Supervisor in April of 2024. Kirsten is a certified Registered Medical Assistant and Registered Phlebotomy Technician. Prior to her work at CCPM, Kirsten was a Medical Assistant at Health Access Network for nearly 8 years. Kirsten resides in the Lincoln area with her husband and two young sons. When not at work, Kirsten enjoys vacationing and camping with her family and friends.

    Kristy Griffin joined CCPM as a Quality Improvement Specialist in late 2022. Prior to her time with CCPM, Kristy worked as an EMT and CNA for many years. Most recently, she worked for PCHC as an MA and Practice Manager. In her spare time, Kristy loves reading and spending time with her husband, three grown children and their dogs.

    Senior Medical Advisor
    Noah Nesin, MD joined CCPM as the Senior Medical Advisor in 2023. He has been a family doctor in Maine since 1986, first in a private, solo practice and then in FQHCs. He has held past medical leadership roles at Health Access Network and Penobscot Community Health Care. Throughout his career, Dr. Nesin has led efforts in practice transformation. He is passionate about evidence-based prescribing work and improving access to care for our community. In his spare time, Dr. Nesin enjoys biking, sailing, pickleball, and gardening.

    Rhonda Scott joined CCPM as a Pre-Visit Planning Medical Assistant in April 2024. Rhonda is a certified Registered Medical Assistant. Prior to her work at CCPM, Rhonda worked at Millinocket Regional Hospital for 24 years. One of Rhonda’s many strengths is her attention to detail and ability to find information; a skill that serves her well in her role. Outside of work, Rhonda enjoys reading, camping and spending time with her family, especially her grandchildren.

    Sarah Fiedler joined CCPM as a Pre-Visit Planning Medical Assistant in early 2023. Sarah earned her Clinical Medical Assisting II certficate at Northeast Technical Institute. Prior to joining CCPM, Sarah worked as a Registered Medical Assistant for 9 years at Millinocket Regional Hospital’s primary care practice. Outside of work, Sarah enjoys reading, gardening, nature walks, and road trips with her family and two dogs.

    Quality Improvement Specialist
    Stephanie Comstock joined CCPM in 2021 as a Quality Improvement Specialist. Stephanie holds a Bachelor Degree in Physical Education. Prior to her time at CCPM, Stephanie worked as a Rehab Technician in a skilled nursing facility and as a Licensed Chiropractor Assistant and Clinical Staff Manager in a chiropractic clinic. Her areas of expertise are HIN Analytics and management of gap closures in our payer portals. Stephanie lives in the greater Bangor area. In her free time, Stephanie enjoys crafting, being active outdoors and hanging out with her Great Dane.

    Tiffany DeMassini serves as CCPM’s Clinical Data Specialist. Tiffany has an associates degree from Bristol Community College. She has taken several online Data Analysis courses and brings several years of experience working in a healthcare setting. Prior to joining the CCPM team in 2023, she worked in the scheduling department at a local FQHC. She has a passion for coding and data science, and brings this enthusiasm to her role at CCPM. In her spare time, Tiffany enjoys reading, painting, and cooking for family and friends.

    Tracey MacDonald serves as CCPM’s HR & Finance Manager. Tracey graduated from the University of Maine Augusta with a degree in Mental Health & Human Services. For thirty years, Tracey worked for a social services agency overseeing various programs and supporting children and families in need. After graduating in 2022 with her MBA, Tracey joined the CCPM team as Business Operations Manager. Diligent and detail oriented, Tracey strives to positively impact our team and member organizations. Tracey resides just outside the Bangor area with her partner Ben. When she is not working, Tracey enjoys traveling, going for walks, and spending time with her family, especially her granddaughter Natalie.

Trudy has extensive knowledge in medical coding which she brings to her role at CCPM. In her spare time, Trudy enjoys reading, crafting, and camping. Trudy’s grandchildren are her pride and joy.
